IT Access forms

WHERE GIVEN THE CHOICE, PLEASE USE THE ONLINE VERSION IF POSSIBLE. THIS IS BOTH SAFER AND FASTER.

The Service Desk will process requests submitted via these forms within a maximum of 5 working days of receipt - please allow for this when requesting accounts/changes.

Please note that the online forms require a valid Kingston login (your normal username and password) as security.

REQUEST TEMPORARY/PROJECT ACCOUNT form

REQUEST Visitor, Contractor or other Affiliate Account

(Use this form if you wish to request an IT account for a member of staff who is not on the payroll system, or a student who is not on the Student Records System. There is a separate form for Collaborative Partners here)

REQUEST CHANGES OR NEW SERVICES form

Request change or access to services

(Use this form to modify/add services to an existing account, such as access to Software, a network service or increased disk/mail quota, or to create/change Noticeboards etc)

Staff requirng IT access who are not part of KU and are based externally:

Please contact your Kingston University administrative contact, who will act as a proxy for the creation of your IT account and will also supply you with the relevant form, and, once the account has been created, your login details.

We have put this procedure in place as a simple security measure.

NOTE: Staff based at KU who are being asked to act as a proxy should send the completed access form to the ICTS Helpdesk. We will return it to you with all the relevant account details to pass on to the external applicant.
